Application Form Instructions
Follow these instructions to save, complete, and submit the protégé application form (Word format). You might want to print these instructions for handy reference.
- Click on the link to the form.
- When given the option to open or save the file, choose Save. Add your company name to the file name (e.g., UCN-21585-MyCompany.doc).
- Save the file to a location on your computer where it can be easily found.
- Navigate to the location of the form, and double click on the filename to open it.
- Use the tab key to move from one fillable field to the next, providing the information requested.
- When complete, save the file. Double check to ensure that the file name includes your company's name.
- Open your mail program, and address an email to mentorprotegeapps@y12.doe.gov (or simply click on the linked email name to open your mail program with the proper address).
- Compose the email message and attach your completed version of UCN-21585 to it. Add your email address in the cc list if you wish.
- Hit the Send button.
