Life Insurance

Basic life insurance costs Y-12 employees $0.195 per $1000 of coverage per month. Supplemental life insurance is available for both employees and spouses. Rates are shown in the table below. All eligible dependent children age 6 months to 19 years of age (up to age 24 if a full-time student) can be covered for $0.89 per month for $10,000 of coverage. Note: Medical premiums for hourly and weekly employees are deducted the first four pay periods in the month. Dental premiums for all employees are deducted once a month. Dental premiums for hourly and weekly employees are deducted the second pay period in the month.

Special Accident Insurance Premiums

    • $0.30 per $10,000 for single coverage
    • $0.58 per $10,000 for family coverage
  • Hourly Employees:
    • $0.41 per $10,000 for single coverage
    • $0.78 per $10,000 for family coverage
