Local Providers

Links to websites are provided for information only. These hospitals may not be approved for use under your medical plan. Check your medical plan for approved hospitals. The Covenant Health website also includes links to many regional hospitals and services and is provided here for your reference.

  • Medco Prescription Drug Plan (CIGNA Open Access and CIGNA POS) —Members of Medco Online Services are able to:
    • order refill mail service prescriptions
    • check on the status of their mail service prescriptions
    • order mail service envelopes and out-of-network claim forms
    • find the location of the nearest network pharmacy

  • Vision Service Plan (CIGNA Open Access and CIGNA POS)—The Member Services number is 1.800.877.7195.

  • Metlife Group Long-Term Care InsuranceImportant Notice:

    Met Life has advised B&W Y‑12 that they have decided to stop offering Long-Term Care insurance policies. If you currently have a Long-Term Care insurance policy through the B&W Y‑12 group plan, no action is necessary. Your policy will remain in effect and payroll deductions for premiums will continue. Your coverage cannot be cancelled as long as premiums are paid on time, although premiums for coverage can be raised. You may continue to make coverage changes according to the terms of the policy or certificate, including participating in inflation protection offers and requests to increase or decrease coverage.

    If you are not currently covered, but are interested in Long-Term Care coverage, you may apply for coverage under the B&W Y‑12 group policy until April 30, 2011. You may apply for yourself, your spouse, your parents and your grandparents up until April 30, 2011.

    Additionally, any new hires who are still within their initial 90 days of guaranteed coverage must submit an application by April 30, 2011 in order to be an on-going participant in the Plan.

    Again, the deadline for submitting an application to Met Life is April 30, 2011. Contact the Benefits Service Center for an enrollment package.
